What is Opioid MME Calculator
The opioid MME calculator converts several opioids into a single figure of morphine milligram equivalents per day, so total exposure can be compared and monitored. Enter the daily dose of each opioid the patient takes and it multiplies by the standard conversion factor and sums them. The total maps to the CDC risk thresholds of 50 and 90 MME per day.

When to use it
A patient takes 60 mg of morphine and 30 mg of oxycodone a day, and you want the combined exposure? Enter both and the total comes to 105 MME per day, above the 90 threshold where the CDC flags higher overdose risk. It is useful for reviewing polypharmacy, planning a taper, and documenting total opioid load.
Frequently asked questions
What do the 50 and 90 MME thresholds mean?
The CDC highlights 50 MME per day as a point to reassess and use caution, and 90 MME per day as a level of substantially higher overdose risk that warrants careful justification. They are guideposts, not hard cut-offs.
Why is methadone not included?
Methadone has a non-linear, dose-dependent conversion to MME and a long, variable half-life, so a single factor would be misleading. It is deliberately left out of this tool.
Can I use MME to switch between opioids?
Not directly. MME estimates total exposure, but rotating opioids requires a dose reduction of around 25 to 50 percent for incomplete cross-tolerance, plus clinical judgement. Do not convert one-to-one.
