What is SOFA Score Calculator

The SOFA score, short for Sequential Organ Failure Assessment, grades dysfunction in six organ systems: respiration, coagulation, liver, cardiovascular, central nervous system and kidney. Each scores 0 to 4, so the total runs from 0 to 24. A higher score and a rising score over time both track with worse outcomes in the ICU, which is why SOFA underpins the Sepsis-3 definition.

How to use

  1. For each of the six systems, pick the row that matches the worst value in the period.
  2. Use the criteria shown, such as PaO₂/FiO₂, platelets, bilirubin, MAP or pressors, Glasgow Coma Scale and creatinine.

When to use it

Assessing a septic patient in the ICU and need a single number for organ failure? Score PaO₂/FiO₂ under 200 on the ventilator, platelets under 50, a MAP under 70 on noradrenaline and a Glasgow score of 10, and the running total climbs into the high band in seconds. It is handy for tracking a trend across days and for applying the Sepsis-3 criterion of a two-point rise.

Frequently asked questions

What is a normal SOFA score?

A baseline SOFA of 0 is assumed in patients not known to have organ dysfunction. Any points above that reflect dysfunction, and the higher the total the worse the associated mortality across study cohorts.

How does SOFA relate to Sepsis-3?

The Sepsis-3 definition calls sepsis a suspected or confirmed infection plus an acute rise in SOFA of two points or more. This tool gives the total; you compare it against the patient's baseline to see the change.

Which value should I enter for each system?

Use the worst value recorded in the 24-hour period being scored. If a lab or measurement is missing, that system is usually scored as 0, which can underestimate the true total.

What total maps to each mortality risk band?

This calculator groups the total into four bands aligned with published ICU cohorts: 0 to 6 is low risk (roughly under 10% mortality), 7 to 9 is moderate (about 15 to 20%), 10 to 12 is high (about 40 to 50%), and 13 or more is very high (over 50%, rising with the score). The colored badge next to the total shows which band you are in. These figures are approximate and describe cohorts, not one patient.
