What is qSOFA Calculator
The qSOFA, or quick SOFA, is a fast bedside screen for patients with suspected infection who may be at higher risk of a poor outcome. It uses three signs, each worth one point: a respiratory rate of 22 or more, a systolic blood pressure of 100 mmHg or less, and any acute change in mental status. A score of 2 or 3 marks higher risk and should prompt closer assessment for sepsis.
How to use
- Enter the respiratory rate in breaths per minute.
- Enter the systolic blood pressure in mmHg.
- Tick the box if there is altered mental status (GCS below 15).
- Read the qSOFA score out of 3 and its risk band.
When to use it
A patient on the ward with a suspected infection looks unwell and you want a quick risk flag? Enter a respiratory rate of 24 and a systolic pressure of 90, and the score reaches 2 straight away, in the higher-risk band. It is a rapid trigger to escalate care, take lactate and cultures, and reassess, rather than a number to act on in isolation.

What qSOFA score is concerning?
A score of 2 or 3 is the threshold for higher risk. It identifies patients with suspected infection who are more likely to have a prolonged ICU stay or die, and it should prompt a fuller work-up.
Is qSOFA the same as SOFA?
No. qSOFA is a rapid three-item bedside screen needing no labs, while the full SOFA scores six organ systems using laboratory and physiological data. qSOFA flags who to assess; SOFA quantifies organ dysfunction.
Does a low qSOFA rule out sepsis?
No. qSOFA has limited sensitivity, so a score below 2 does not exclude serious infection or sepsis. Clinical concern should always override a reassuring score.
