What is Wells Score Calculator (DVT)

The Wells score estimates the pretest probability of a deep vein thrombosis from ten clinical findings. Nine of them, such as active cancer, a swollen leg or a previously documented DVT, each add a point, while an alternative diagnosis that is at least as likely subtracts two. In the widely used two-tier model, a total of 2 or more means DVT is likely and below 2 means unlikely.

When to use it

A patient turns up with a swollen, tender calf and you want to structure the work-up? Tick localized tenderness, a swollen entire leg and calf swelling over 3 cm, and the score reaches 3, in the DVT-likely group that points towards imaging. In the unlikely group, a negative D-dimer can often safely rule out thrombosis without a scan.

What does the two-tier Wells score mean?

In the two-tier model, a score of 2 or more means DVT is likely and warrants imaging, while a score below 2 means DVT is unlikely and can often be combined with a D-dimer to safely exclude it.

When do I subtract two points?

Subtract two only when an alternative diagnosis is at least as likely as DVT, for example a clear cellulitis or a ruptured Baker cyst. It is the single negative item in the score.

Does a low Wells score exclude DVT on its own?

No. A DVT-unlikely score lowers the probability but is usually paired with a D-dimer, and imaging is still needed to confirm or exclude thrombosis when suspicion remains.
